A targeted case manager assists each person with developmental disabilities and his or her support network to identify, select, obtain, coordinate and use both paid services and natural supports that may be available to enhance the person’s capabilities and preferences as outlined in a person-centered support plan.
Whether it is going to a park with friends, attending country music concerts or working on a project in the day services center, preferred activities are identified and given high priority in the individual’s support plan.
Case managers at CCL help make this person-centered planning possible. They are professional advocates who coordinate all services and keep everyone - especially the parent or guardian - informed. The case manager works with behavioral consultants, direct support professionals, nurses, physical therapists, speech and language pathologists, occupational therapists, teachers and dietitians when needed to insure each individual plan is followed.
CCL can provide case management to people who do not receive other services from the agency.
Each person receiving services and the person’s guardian have the right to choose the case management provider to assist them. CCL is licensed to provide case management services in Cowley and Butler counties.
